Please be aware that as of December 1, 2023 The City of Rahway Department of Health will NO LONGER be accepting cash. 


All fee's must be paid by Check or Money Order.

Birth, Death, Marriage, Civil Union & Domestic Partnership

The Rahway Department of Health records ONLY births, marriages and deaths that occurred with the municipality. We only issue Long Form Records (parents name/names are listed on the record)  unless a Short Form is requested by the applicant. All records that are issues are original certified copy's with the raised seal.

Non-Genealogical Records (current events) are:

  • All domestic partnerships and civil unions
  • Births occurring within the last 80 years
  • Deaths occurring within the last 40 years
  • Marriages occurring within the last 50 years

Important Information to know:

  • You need to have been born in Rahway, New Jersey in order to obtain a Certified Birth Certificate
    • Your Mother’s Maiden Name
    •  Father’s Name (if father is listed on original record)
    • Date of Birth
    • Full name at the time of birth
  • You need to have been Married in Rahway, New Jersey in order to obtain a Certified Marriage Certificate
    • Your full name at the time of birth
    • Your spouses full name at the time of birth
    • The date of marriage
  • The Deceased must have passed away in Rahway, New Jersey in order to obtain a Certified Death Certificate
    • The deceased full name at the time of birth
    • The deceased mothers maiden name
    • The deceased fathers name
    • The date of death

If you were Born, Married or the Deceased passed away in a different Municipality, you must contact that Municipality, or The State of New Jersey Department of Vital Statistics.

Apply By Mail:

You will need to mail in:

  •  A photo copy of a valid photo ID (not expired)
  • The competed NJ Vitals Application with all the required information listed above
  • Check or Money order made out to The City of Rahway. ($10.00 per certified copy)
  • The address that you would like us to mail the Certificate back to

These are Original Certified Copies with the raised seal

We will then mail the Certificate to you via USPS. We will issue the Certificate the same day we receive the application, required documentation and fee. If you would like the Certificate overnight; you must include an overnight envelope with your application, required documentation and fee.

Valid photo driver's license or photo non-driver's license with current address. OR
Valid driver’s license without photo and an alternate form of ID with current address. OR
Two alternate forms of ID, one of which must show the current address

  • The NJ Vitals Application completed with all information listed above
  • Check or Money order made out to The City of Rahway. ($10.00 per certified copy)

    • Utility bill (within the previous 90 days)
    • Bank Statement (within previous 90 days)
    • W-2 or tax return for current or previous year

For those who are homeless, they can provide identification from a social worker or homeless shelter coordinator.

For those who are incarcerated, they can provide legal imprisonment, conviction papers or release documents that include the name, social security number and all possible aliases used in the past or identification from a prison/probation official.
